NAVAL UNDERWATER MEDICAL INSTITUTE OF PAKISTAN

History of Hyperbaric Medicine in Pakistan :

Although Hyperbaric Medicine has been Present since the late 1970 ,s in Pakistan Navy and is mainly Used for the Purpose of Navy Operations. Now this medical Specialty is Treating more than 300 Diseases and Has a Text book and more than 100 Reference Books.

Naval Hyperbaric Medicine of Pakistan is Associated with Special Services Group of Pakistan Navy , also Known as Navy Commando,s Identified as PNS Iqbal Unit in Pakistan Navy, Situated in Karachi.

Hyperbaric Medicine is also used in PNS Mehran Aviation unit of Pakistan Navy, where the Pilots of Pakistan Navy are Exposed to training in Sea Based Naval Operations of Search rescue and Missions over Sea of Naval Aircraft .

Hyperbaric Specialists are those who are Selected from Pakistan Army medical Corps . Known as AMC . AMC is Medical Core of Doctors as officers and Nursing Personal Serving in Pakistan Defense Forces in Army, Navy , and Air Force .

The Naval medical Directorate, Situated in Islamabad is Manned by Pakistan Army medical Corps Doctors and it has Multiple Medical units like Hospitals etc Functioning under it .

There is a Medical University called Bahria University that also has its own Faculty of Medicine under Medical Directorate. Bahria is Urdu Language word for Navy .

This Naval medical Directorate is Situated in Pakistan Navy Head Quarters in Islamabad. Because of Operation of Pakistan Navy in Deep Sea , Tactical and Scuba Diving Operations Associated with Submarine , Special Services Commandos , the Department of Hyperbaric Medicine is very Much Needed in Pakistan Navy , Air force and Army .

Hyperbaric Medicine in Pakistan Progressed in late 1980,s when Pakistan and USA had very Cordial Relationships and it was Partner in Geo Strategy in Asia and in Afghanistan where they were fighting the war against Communism .

USA and Pakistan helped the Hyperbaric Medicine in order to Facilitate, the Pakistan Navy to do the Submarine and Naval SSG Special Services also Known as Naval Seals Operations that Operated the Submarine Warfare and also the X -Craft Spy and Tactical Under water Submarine Crafts that Operated under the Water.

Pakistan Navy Sent in the Doctors who were from Army Medical corps and they Trained them in Naval Seals / Navy SSG Operations in Karachi at Pakistan Navy Facilities and when they Got Trained they were Taught Hyperbaric Medicine in Institutes of USA in Co-operations with Bethesda Washington DC , and its Associated Hyperbaric Training Institutes along Side with USA Doctors of its Navy and Army . Some of them also did Navy Seals courses .

Due to gracious help of US and its Medical Corps Pakistan Navy and Army Medical Corps of Pakistan was able to Establish the Naval Medical Institute in Pakistan Navy alongside of Pakistan Navy Special Services Group / Naval Seals .

NUMI , or Naval medical Institute came into being and Hyperbaric Medicine Started and two and three Batches of Doctors went to USA and some of them of continue to go even now .

Due to this USA started the Hyperbaric Medicine in early 1980,s and Pakistan Immediately did that in Late 1980,s really at the Same time.

NUMI , Naval Underwater and Hyperbaric Medical Institute at PNS Iqbal:

In Pakistan Navy , Hyperbaric Medical Specialist Certificate course is done in , Naval Underwater Medical Institute NUMI , in Karachi , that has senior Instructors and in Hyperbaric Medicine who Impart training in Hyperbaric Medicine .

These Senior Instructors and Consultant Doctors are Trained in Advanced Hyperbaric Medicine , from USA and Australia in Advance Hyperbaric Medicine course , and they are than Ready to Impart training to the New Doctors for Hyperbaric Medicine .

The Senior Instructors have More than 5-10 Years of Experience in Hyperbaric Medicine as a Specialist and have also Served for At Least 1 Year on a Deep Diving Ship also Known as Mine hunter in Pakistan Navy as it does Diving Operations of Deep Sea for Sea Based Mines and Weapons .

For Example I served for more than a Year on PNS Munsif a Mine Hunter ship that was doing Deep sea Diving and this a Special Ship that it has a Hyperbaric Chamber on Board also Known as TRCS Transportable Re-compression Chamber System , that joins with Bigger Hyperbaric Chamber via NATO Flange.

Only Mine Hunters has Hyperbaric chambers on Board and no other ship has Hyperbaric chambers on Board and this Kind of training can only be done on Mine Hunters.

PNS Shifa :

PNS Shifa , is A class of hospital with more than 1000 Bed and is based in Karachi and is Tertiary care A Grade Hospital with a lot of Specialties and Hyperbaric Medicine also comes under it and it has a Training Section that Trains the Nurses and Doctors as well and it is Under the Naval Medical Directorate and the Bahria University.

Pakistan Navy started PNS Shifa as a Hospital and a training school named as sick berth attendants in 1953. It was re-designated as “Medical Assistant Training School” in 1970.

The name of Pakistan Navy Nursing School was conceived in 1982 when the Female Nursing Cadets and Armed Forces Nursing Services (AFNS) trainee officer were also attached for various Professional courses.

Recently; keeping in view the role of hospital / school in imparting post-graduation training to medical officers, the name has been changed to Pakistan Navy Medical Training School.

PN Medical Training School is entrusted with the primary task of training to medical branch personnel of Pakistan Navy who serve on board ships / establishments to look after the patients.

This school is recognized by Sindh Nurses Examination Board & Sindh Medical Faculty and providing training facilities for General Nursing diploma course of 03 years duration and one year Operation Theater, Intensive Care and Midwifery specialization courses for AFNS officers.

The Hospital and its Training school also conducts house job, FCPS-II/DCPS (post-graduation) and Underwater Medicine Training.

Training School / PNS SHIFA is recognized by Pakistan Medical & Dental Council and College of Physicians and Surgeons Pakistan for House Job, Post-Graduation training in almost all the Disciplines of Medicine and Surgery and

Training in these Specialization is also being offered to Civilian Doctors in addition to Commissioned Medical Officers of Defense Forces.

Underwater Medicine Course is being offered to Defense Services Medical Officers of Allied Countries as well.

Hyperbaric Medicine Specialization Course :

Hyperbaric Medicine Training is About 2 Years Total which has following Components :

6 months of Theory Part at NUMI Naval under water Medical Institute, under Bahria Naval University Department of Medicine and Naval Medical Directorate .
Plus 6 Months of Hyperbaric Medicine Training at PNS Shifa 1000 Bed Post Graduate Medical Institute , under Bahria Naval University Department of Medicine and Naval Medical Directorate , Clinical Aspects of Hyperbaric Medicine and its training under Senior Instructors.
1 Year diving and Practical Training at PNS Iqbal Naval Special Service Facility.
1 Year diving and Practical Training on Board a Diving Mine Hunter ship that has Active Diving Operations

Total of 3 Years of Course is done before a person is Fully Qualified as Hyperbaric Medical Specialist is Requirement in Pakistan Navy with both Theory and Practical Aspects.

Certificate of Hyperbaric Medicine Specialization is Issued by the Naval medical Directorate of Pakistan Navy in Islamabad , of 6 Months Duration only that only Signifies only the Theory Aspects and does not Mention the Practical 2 years and Six Months of Training that is a Flaw with Certificate ,does not Mention the, 6 months of Hyperbaric Training at PNS Shifa Hospital and also Practical Training of 2 Years that are Pre-requite for being Hyperbaric Medical Specialist which is an Anomaly that has to be Corrected the Certificate Issued by Pakistan Naval Medical Directorate , is of level -2 in International Classification.

The Level -2 Hyperbaric Medical Certificate that is Given by Naval medical Directorate of Pakistan Navy , is also Not Mentioned in PMDC Pakistan Medical and Dental Council List as well as Second Anomaly that Also needs Correcion, as the Pakistan Navy and AMC , Army medical Corps of Pakistan Army did not Service Pay the Fee , of PMDC for Recognition of Naval Medical Directorate and NUMI , that are Bestowing the Certificates.

For Instructors Advance Hyperbaric Course is done abroad , in USA and Australia when the Senior Hyperbaric Medical Specialists are sent by the Pakistan Navy after they complete the and Plus 3 Years of Training Post Qualification .

After this Training in USA and Australia the Doctor is now Consider Fit for being a Qualified Hyperbaric Specialist and Senior Instructor and he/she can be:

Examiner of Hyperbaric Medicine for Divers and Submariners
Instructor at NUMI , Naval Medical Under Water Medical Instructor.
Licensed to Practice as Specialist as Hyperbaric Medical Consultant

In Pakistan Level -3 , FCPS Hyperbaric or Master or PHD Degree is not Available as of yet , from Pakistan .

Internationally Advance Level -3 , Courses are not Available in USA Europe, Asia Africa, or Oceania ( Australia / New Zealand ) in any English Speaking Countries , Level -3 Degree, is not Available in any English Speaking Countries.

Hyperbaric Medicine in Pakistan Army for High Altitude Adventures :

Hyperbaric Medicine was Utilized in Late 1980,s in Pakistan Army for High Altitude Adventures and Early 1990,s when the War on Kargil which is highest Battle filed in the World and Pakistan and India has a cold War like Situation there since then and Active Warfare is being engaged on the Roof top of the World .

The Army Medical Corps Doctors were Involved in Solution of Problems of High Altitude Sickness in the form of HAPE and HACE known as High Altitude Pulmonary Edema and High Altitude Cerebral Edema .

The use of Flexible chambers were done to Save countless Lives of Army Personal involved in War Fare on the Roof top of the world as 150 Mountains exists that are above 7000 meters over there which is like 7 Km in Height from Sea Level .

Hyperbaric Medicine Was used by the Army medical corps Doctors for the Saving of Lives and Currently it is also being used in Mountaineering .

As Hyperbaric Chambers are saving Lives of Mountaineers and also , Enabling them to climb the Mountain without using Oxygen Cylinders which are not helpful in Rare Atmosphere .

The Hyperbaric Chambers provides them at Base camp to Boost up the Oxygen Levels and Boost them to climb the Mountains and conquer the Challenges .

Pakistan Hyperbaric Specialists are Working in the Following Foreign Countries :

These Doctors are Working on Deputation and in Uniform mostly on Loan from Pakistan Army and Pakistan Navy and some Retired Doctors from Pakistan Defense Forces are also Working in Gulf countries.

Diving Chamber

Diving Hyperbaric Chambers :

Hyperbaric medicine has been Developed as Deep sea medicine , because of Endeavors of Diving in the sea and Development of Diving Equipment that enabled to Dive deeper than 30 meters, under pressure of the sea and resulting Bends and chokes happening called Decompression Sickness .The First Hyperbaric chamber came into being in order to treat that .

The Diving Chambers is different from other chambers because of Higher Pressure than the Medical Chambers .

Range of Operation 0-10 bars Or More like even up to 100 Bars has been in used in for Different Purposes .

Diving Chambers are Used in Oil and gas Exploration, in Navy , Military, Aviation and Oil and Gas. For Diving Emergencies , Diving Operations , Simulated Dry Dive inside of Hyperbaric Chamber depending on how Deep the Diver has Dived .

Divided into Following Types :

1. Air Compression System.

2. Saturated Gas System.

3. TRCS ( Transportable Re-compression Chamber System).

4. SPHL ( Self Propelled Hyperbaric Chamber Life Boat ).

5. SR Submarine Rescuers.

6. Flexible Helicopter based Hyperbaric Chambers

Diving Hyperbaric Multi place chambers are used by People in Navy , Coast Guard Facilities , Hospitals , or Research Centers like Medical Universities , Further more as Multi Place Diving Chamber are found near the Seas , where there is a lot of Diving Activities are occurring like in the Navy and Special Forces units of Navy or the Army.

There are two Types of Control Systems :

Fully Computerized Control System
Manual Control System

Computerized Control System :

In Fully Automatic HBOT Chambers Software's Control the Multi Place Chamber and its Prevents the Barotrauma by Controlled Pressure Increase and Depth Control , and hence is more Beneficial for the Patients and it is more conformable to run and it corrects the Mistakes of Operator who can Injure the Ears and Sinuses of Patients by controlling the Depth and Pressure Control .

Manual Control System :

In Manual Controlled HBOT Chambers Operator Control the Multi Place Chamber via a Handle like Throttle shaped like Airplane Jet Control , hence it is more Direct Control and Mistakes are not Corrected by the HBOT Chamber. A well trained Operator is Required for Operation and running of this Chamber with Sufficient Experience .

Gas used to Pressurize Mono Chamber :

Diving Multi Place Hyperbaric chambers are Classified according to Gas used for Pressurization :

Air Pressurization compression system
Saturation Gas used as compression system

Air Pressurization System :

However the Air Compression Multi place Chambers are used for Diving and Have Pressures up to 10 Bars usually and also manufactured by Different Companies

In the Air compression oxygen mask or oxygen helmet is used to deliver the oxygen to the patient under taking the treatment .

Saturation Gas Pressurization System :

In the Saturation Gas compressor system of Diving chambers instead of Air that has 21 % Oxygen Gas , a Different type of Gas that is Inert is used for Compression of Divers and that is done so that there it is safer .

Saturation Gas Chambers used in Heliox Gas Compression

The Inert Gas that is used for Compression is Either "Heliox" Mixture of Helium Gas and Oxygen , Saturation diving is a diving technique that allows divers to reduce the risk of decompression sickness ("the bends") when they work at great depths for long periods of time. Saturation divers typically breathe a helium–oxygen mixture to prevent nitrogen narcosis.

In saturation diving, the divers live in a pressurized environment, which can be a saturation system on the surface, or an ambient pressure underwater habitat when not in the water. Transfer to and from the pressurized surface living quarters to the equivalent depth is done in a closed, pressurized diving bell. This may be maintained for up to several weeks, and they are decompressed to surface pressure only once, at the end of their tour of duty.

Saturation Gas Chamber

By limiting the number of decompression in this way, the risk of decompression sickness is significantly reduced, and the time spent decompressing is minimised. It is a very specialized form of diving; of the 3,300 commercial divers employed in the United States in 2015, only 336 were saturation divers.

Usually Done on work on Deep Sea Oil Rigs and Salvage Operations and also Military Dives that are highly Specialized

TRCS Transportable Re-compression Chamber System :

Transportable Re-compression Chambers System or TRCS in short is a Diving Chamber system that is used for Rescue of Divers and is used for Transport of Patient from Site of Diving Accident to the Diving Hyperbaric Chamber .

TRCS Diving Chambers

As the Diving Chamber is Far Away and TRCS is Efficient way of transporting the Patient to the Main Chamber. After the Main Chambers is Reached the TRCS Attaches to the Main chamber and then Patient is transferred from the TRCS to the Main Diving Chamber to be Treated by the Trained and Highly Professional Team of Medical Doctors and Hyperbaric Nurses trained in Hyperbaric Operations .

TRCS Chambers Being Attached with Main Chambers

The TRCS Attaches to the Main Chamber by the way of NATO Flange , which is an Attachment that is Designed with specific Measurements and Shape , and this is International Standard of the Shape of the Adapter . The Function of this Adapter is such that TRCS can Attach to any Main Diving Hyperbaric Chamber .

TRCS is used for following Purposes that is , it can be Carried away in air sea and Land like

1. Land vehicle like a truck or Van Pickup etc
2. Air in a Plane or a Air Helicopter
3. Or on a Ship .

For Diving Operations and when needed can be Transferred back to Main Diving Chambers to be used for Transport and has capacity of only one man or Max 2 who can be an Attendant etc

SPHL Self Propelled Hyperbaric Life Boat :

Self Propelled Hyperbaric Lifeboat or SPHL , is A Hyprbaric System that is incorporated into a Boat and then it is fast Boat that can Reach the place of Diving Accident very quickly and rescue the Diver or Patient .

The running Cost of SPHL is a lot cheaper , than a Ship based Hyperbaric System and is very cheap to run and Maintain as Compared to sending a whole ship, for Hyperbaric Rescue .

SPHL is being used for over the Surface water Rescue as compared to SR submarine Rescue Vehicle , below the the water Operations .

SR Submarine Rescuer :

Submarine Rescuer , is a Specialized Vehicle used in Under Water Operations and Hyperbaric Treatment , that is used for Transfer of Under Water Patent who cannot be Brought to the Surface for Treatment Immediately .

Submarine Rescuer

SR or Submarine Rescuer has on Board Hyperbaric Chamber on Board like the SPHL and is used to Transfer the patient from a Submarine into the SR Vehicle and then Patient is then transferred to main chamber from there or to TRCS and then to Main Chamber .

Flexible Heli and Vehicle Based Chambers :

Helicopter Based Hyperbaric Diving Chambers has been gaining Popularity as they are Light weight and can be used as TRCS or Main Chamber Life saving Systems .

The Application of such System is done in Hyperbaric medical and Diving Operations as Helicopter is fast and Efficient way to transport Patients and this chamber can be Removed from the Heli to Transfer the Patient to the Main Chamber as well .

History of Hyperbaric Medicine in Saudia Arabia :

Saudia Arabia has been using Doctors from Pakistan Navy for Hyperbaric Medicine Needs of Navy and the Coast Guard.

The Saudi Air force and Army has also been Using the Hyperbaric and Aviation Medical Specialists from Pakistan for the Defense Needs of Saudia Arabia and Also the Special Services Group of Saudi Navy , also Known as Navy Commando,s also use the Pakistani Doctors .

Saudia Arabia has also recently sent its own Doctors to USA and has trained some of them and they work in Coast Guard Hospital and also in Riyadh and Jeddeh Tabuk Area of Saudia Arabia .

The Saudi Defense Forces Doctors of Saudia :

USA and Pakistan helped the Hyperbaric Medicine in Saudia Arabia and they were able to get some Doctors to be Hyperbaric medicine Qualified like Saudia Dr Yousaf al Oufi who is Director of Hyperbaric Operations in Saudia Arabia Jubail Naval base and he is In charge and has Degree in Hyperbaric Medicine from USA .

Saudi Navy to do the Submarine and Naval SSG Special Services also Known as Naval Seals Operations take place from Jubail Naval Base , and also the Submarine Warfare and X -Craft Spy and Tactical Under water Submarine Crafts that Operated under the Water, also Take place from Jubail .

Multi Place HBOT Chamber

Multi place chambers are used by People in Hospital , or Research Centers like Medical Universities or in Stand alone Emergency centers that are associated with a lot of Satellite Specialized Medical clinics or Hospitals .

Hyperbaric Chamber are also being used in Plastic Surgery and Hair transplant , as well other Surgeries that required healing like Cardiac and Surgical orthopedic ones

Further more as Multi Place Diving Chamber are found near the Seas , where there is a lot of Diving Activities are occurring like in the Navy and Special Forces units of Navy or the Army.

The Stand Alone Emergency Dealing Centers like the Coast Guard and the Navy also have their Own Multi place Hyperbaric Chambers .

The Oil and gas Drilling Industry also Deal in Hyperbaric Chambers and they are used in Deep Sea Diving and off Shore Drilling Operations

The Multi place Chambers are also found in Aviation and Space Technologies like in NASA and European Space Program's and in Future Pakistan can also use this for its Astronauts and Pilots .

These Chambers Although vary in Price that is not so cheap for third World countries .

The Chambers are Classified in North America by FDA and NFPA, that is Food and Drug Authority and National Fire Protection Agency

FDA USA Classification : Class 2 Chamber

NFPA USA : Class A

Hence it is Class 2 A Product

Both The FDA and EC in USA and Europe does Recommend the Hard Multi Place Chambers as worthy of Use for Regular Treatment of Hyperbaric Therapy,

Multi place Chamber Types :

According to External Shape it has following types

A. Cylindrical Multi-Place Spherical ( Cylindrical with a Bases )

CYLINDRICAL MULTI CHAMBER

there is no Base at the bottom and a Special base has to be made at the bottom so that Chamber can be housed there either as trench or as a Plat Form so that Chamber is Securely Placed there .

B. Omega Multi-Place ( Does not Require a Base to Support Itself )

OMEGA MULTI PLACE HYPERBARIC CHAMBER

Has a Flat Base at the Bottom which supports the Chamber and does not Require the Making of base for its installation and is very Easy to Install and Supports itself on a Flat ground and is very Good as Manufactured by Hipertech in Turkey who has the Patent for the Chamber .

C. Quadro Multi-Place (Rectangular)

QUADRO CHAMBER

Rectangular Better system for lying down , Non Ambulatory Patients to go in via a hospital Trolley and Easy for Disabled Patients with sliding Doors or Hinge Doors.

Quadro has option for treating Lying Patients better than sitting Patients and it helps in healing them with help of qualified Nurse inside of the Chamber .

Gas used to Pressurize Mono Chamber :

Multi Place Hyperbaric chambers are Classified according to Gas used for Pressurization :

AIR Pressurization compression system
OXYGEN Pressurization compression system

Air Pressurization System :

Just like the Multi place chamber has a AIR pressure and compression system , as AIR has 21 % oxygen only and rest is Nitrogen 78% an inert gas That does not take part in combustion or explosion , and hence safer then Oxygen which is highly combustible .

However the Oxygen Compression Multi place Chambers are also manufactured by Different Companies The Air Compression Multi place chamber is more expensive than the Oxygen compression system in price .

In the Air compression oxygen mask or oxygen helmet is used to deliver the oxygen to the patient under taking the treatment .

Oxygen Pressurization System :

In the other pressure compressor system where the oxygen is used for treatment although cheaper change in price and no oxygen mask is used for the patient and he / she lies just like that in the chamber.

But there are drawbacks to This.chamber as it can cause an explosion as pure oxygen is being used for treatment of the patient . Pure oxygen is 100% saturation . This chamber is relatively more dangerous than the Air compression system in mono chamber and most of Mono chambers have this system all around the world and Air compression in Mono place HBot chamber is in un common .
