METHODE

SARS from Covid-19 , Critical Patient Progressive Hypoxia that was un-corrected with Ventilators and ECMO , due to Inflammation of Lungs and Severed Secretions .

HBOT Cleared of SARS Corona Virus , In Matter of just 4-5 sessions, each consisting of 60 Minute at 2 ATA Pressure was used.

Hyperbaric Chamber Proved to be very Powerful Therapy to remove Progressive Hypoxia caused by SARS .

The Hyperbaric Therapy proved to be Decider between life and death , Providing a solution to Failure of Organs.

The Patients responded to Treatment in the , First Session even ,

HBOT proved that even 2 session were enough to save the Lives of Patients having SARS .

The Research Paper, demonstrated that critical Patients after taking 5 session are devoid of symptoms of Fever , cough and Breathlessness

CT Scans of Chest and lungs proved the Point and Partial Pressure of Oxygen also Increased.

Even Having Normal Pulmonary Function after Covid-19 , Imbalance between the Oxygen Demand and supply lead to SARS Hypoxia . Leading to Inflammations of Lungs Due to SARS .

SUMMARY

Background

Patients with COVID-19 present a pneumonia with severe lung involvement, large inflammatory component and structural damage at different levels, such as hemoglobin . This global hypoxia caused multi organ damage. The WHO said that "oxygen is the major treatment intervention for patients with covid-19. The most powerful oxygen therapy with the greatest capacity to supply oxygen that we knew was the hyperbaric chamber since it allows a non-invasive contribution of O2 to 100% and also added the biological effects of increased pressure. Here we show the outcome of COVID-19 patients treated with HBOT

Method

Seven patients diagnosed with COVID-19. The initial objective was to treat clinically compromised patients, with a poor response to the base treatment and risk of worsening, needing ICU admission and connection to a ventilator. We could see the evolution of the analytical parameters: lymphocytes, ferritin, fibrinogen, D-dimer and CRP. We analyzed the oxygen saturation and the need for external oxygen supply.

Findings

All the patients tolerated the treatment very well. They were feeling better, the general message was of subjective improvement. The general trend was an improvement in O2 saturation accompanied by a reduction in the need for external oxygen supply. Tendency of the majority of patients to stabilize / improve the analytical data respect to the status of admission and / or start of treatment. The resuls suggested HBOT could improve/stabilize clinical evolution in COVID-19 patiens even help them to recover

CONCLUSION

Understandably we are aware that this is a short and heterogeneous sample that does not allow us to draw definitive conclusions and, of course, it is not our intention to issue dogmas in this regard.
However, what has been seen in these patients and what other authors tell us suggests that treatment with hyperbaric oxygen therapy can promote aerobic metabolism, correct hypoxia, improve perfusion and oxygen supply to tissues, reduce the component inflammatory etc.

It does not consist of converting the HBOT into a single treatment, nor of replacing the medical treatments that are being administered and studied, but rather about incorporating this therapeutic possibility into the usual treatments of these patients.

Given the limited experience and clinical instability of the patients, we believe that these treatments should be performed in a hospital environment and always with equipment that meets the basic requirements of providing 100% O2 and high pressure, since is pressure and not another variable which makes the availability of free O2 in plasma multiply by much.

Although the situation has improved compared to previous weeks, we are still fully immersed in the pandemic and we do not know the evolution this may have over time. We appeal to our colleagues who have these equipments to treat patients with COVID-19 and to the health authorities to provide this type of equipment to health institutions to continue treating patients, who are still many, and to be prepared for the possible needs that may arise in the future.

Iran after Hearing about the Good Results of Chinese Doctors Research also Decided to do Hyperbaric Medicine Treatment , for the Patients as Iran has got a Severe Problem and Large number of Critical Patients .

Increases the Body Oxygenation in the Tissues :

Hyperbaric Chamber and Ventilator are both Oxygenator Devices ,
Ventilators are Normobaric System as they act at Sea level known as 1 ATM
Hyperbaric Acts on Greater then Sea Level Pressures which can range up to 4 ATM or more especially When Treating Diving Diseases called Decompression Sickness

Hyperbaric oxygen Difference between the 2 , is that Hyperbaric Chamber Increases Oxygenation via Pressurized Environment while the Ventilator does it via Non Pressured Environment but with Positive Pressure Assisted Breathing .

Hyperbaric Chamber is known as Device that can Load Oxygen in the Body without the Need of Even the RBC / Blood as it can Load oxygen Directly by Loading it in the Plasma or Body as 70% of that is water.

Hyperbaric Chambers can Treat Diseases and the Ventilators can just do Assisted Ventilation.

Oxygen acts as an antibiotic by impairing the bacterial metabolism. The effect of HBO is not selective but covers a broad spectrum of gram positive AND Gram negative organisms, It is most effective in anaerobic infections.

HBO produces free radicals which are toxic to the microorganisms. Oxygen has a direct bactericidal or bacteriostatic effect equal to that of some antibiotics especially on Anaerobic Infections .

HBO improves the phagocytosis, which is impaired by hypoxia,

Hypoxia impairs the immune mechanism of the body, whereas HBO improves it.

e.g., alpha-toxin of C. perfringens or detoxifies the oxygen-labile exotoxins, e.g., theta-toxin of C. perfringens of Gas Gangrene

OFF LABEL USE OF HYPERBARIC OXYGEN THERAPY

(The International Hyperbaric Medical Association has worked extensively with government regulatory bodies including the Food and Drug Administration and the Centers for Medicare and Medicaid Services – the IHMA‘s efforts allowed diabetic foot wounds to be approved as the first new indication for hyperbaric oxygen in 18 years- 2003)

POSITION STATEMENT

The International Hyperbaric Medical Association (IHMA) supports the use of hyperbaric oxygen under prescription and oversight by a physician for any indication where a physician finds that the condition is related to a hypoxic event or other conditions where the underlying pathophysiology will benefit from saturating the patient’s tissues with oxygen under pressure.

The core and essence of the conventional practice of medicine has been a physician’s creative use of medications, devices, and therapies that in the best knowledge of that physician has the possibility of treating a patient’s disease. In the 1950’s this long-standing international tradition was renamed “the off-label” practice of medicine in the United States (only). The IHMA supports, defends, promotes, and encourages the off-label practice of hyperbaric oxygen therapy.

DEFINITION OF LABEL BY FDA-FOOD AND DRUG ADMINISTRATION USA.

The term “off-label” was birthed with the creation of the Food and Drug Administration (FDA). As all medical drugs and devices to be used in the United States henceforth required FDA approval, manufacturers submitted applications for approval of their drug or device and the FDA subsequently labeled the drug or device for one or more uses (indications).

This is called the FDA label. Once approved or labeled the device or drug could be used by any physician in any manner to treat any disease or patient, i.e., the practice of medicine. The practice of medicine in the United States thus was divided between on-label and off-label.

OFF LABEL USE IS ETHICAL AND LEGAL

The off-label practice of medicine is ethically underpinned by the Hippocratic Oath which states that as a physician- in the classic version “…I will apply dietetic measures for the benefit of the sick according to my ability and judgment; I will keep them from harm and injustice” or in the modem version, “I will apply, for the benefit of the sick, all measures which are required.”

The ethics of off-label hyperbaric oxygen therapy (HBOT) were strengthened and reaffirmed by the findings of the Ethics Task Force of the Undersea and Hyperbaric Medical Society in 1998.

The Ethics Task Force found that the off-label use of HBOT was entirely ethical, and a scholarly rendition of this opinion by the two bioethicists was published in journal Undersea and Hyperbaric Medicine.

Hyperbaric physicians in the United States can offer their patients the benefits of HBOT for a wide variety of conditions for which their international colleagues also prescribe HBOT.

Furthermore, the legality of treating off-label was established as an important part of the practice of medicine in the USA 26 years ago in United States v. Evers, 453 F. Supp. 1141, 1149 (M.D. Ala. 1978).

In conclusion, the off-label use of HBOT is the ethical application of the principles of the Hippocratic Oath buttressed by recent bioethical research and legal rulings. And as previously stated, the IHMA supports, defends, promotes, and encourages the off-label application of HBOT by physicians.

VENTILATOR AND HYPERBARIC CHAMBER ARE SIMILARLY LABELED

When a ventilator was first approved by the FDA, it had a list of indications with it. However, upon further reflection, it was recognized that the need to get oxygen to someone who could not breathe without assistance did not need an indication, in other words, the requirement to list all potential uses and indications was waived for this device.

The IHMA’s discussions with the FDA on this topic lead the Environmental Tectonics Corporation to submit a 501 (k:) for delivery of hyperbaric oxygen under a physician’s prescription without indications listed (6/26/2002).

This means that the FDA has taken the first step in putting hyperbaric chambers in the same class of devices as ventilators – a device that requires no indications to be listed.

When a drug or device is approved for marketing by the FDA, it normally has to state a list of indications of use.The list would include what diagnoses this drug or device would be used to treat.

When the drug or device is used for a particular diagnosis that does not appear on the original list then for that indication it is considered “off-label”. (Device Labeling Guidance, FDA Guidance Doc. No. G-91, pt. III May 8, 1991)

Off-label uses are neither risky nor investigational; they constitute the traditional practice of medicine that has existed long before the FDA.

OFF LABEL MEANS THAT FDA IS SILENT ON THIS MATTER AND DOES NOT MEANS IT IS ILLEGAL OR UN -ETHICAL

The off-label designation by the FDA is simply a term they use to mean they are silent on the indicated use.

The off-label use of a drug or medical device may be less risky than the approved indications. All drugs and devices contain inherent risks. The labeled indication may just be the one that had the strongest evidence for approval or, in the case of the 13 FDA labeled indications for HBOT, indications, the result of accepting a list without the FDA insisting on scientific due diligence.

In their article “FDA, Off-Label Use, and Informed Consent: Debunking Myths and Misconceptions” Beck and Azari state the following: there is a belief that all off-label treatment is ipso facto “investigational” or “experimental.”

It is an accepted principle that once FDA determines that a drug or device can be marketed, a physician’s discretionary use of that product (the practice of medicine) is not restricted to the uses indicated on FDA-regulated labels.

Off-label use is widespread in the medical community and often is essential to giving patients optimal medical care, both of which medical ethics, FDA, and most courts recognize.

Even so, the public (and an occasional court) mistakenly presumes that all off-label treatment is investigational or experimental. (Beck, James, and Azari. “FDA, Off-Label Use, and Informed Consent: Debunking Myths and Misconceptions.” Food and Drug Law Journal, 53 (1998): 71- 104).

The FDA considers a drug or device “investigational” once the drug or device has been submitted for FDA approval. It then must go through three phases of clinical testing known as the investigational new drug (IND) process. If the IND testing proves a drug or device is safe and effective then the manufacturer would submit a new drug application (NDA).

After the FDA approves an NDA for a device or a drug it then can be marketed by the manufacturer for the indications for which it was approved. The point is that approval means it can be marketed (by the manufacturer) for those uses only.

OFF LABEL CAN BE PRESCRIBED LEGALLY BY PHYSICIANS BUT NOT MARKETED BY COMPANIES AND MANUFACTURERS OF HYPERBARIC CHAMBERS IN ADVERTISEMENTS ACCORDING TO FDA.

A device or drug can never be marketed for off-label uses (by the manufacturer), but can be prescribed for off-label uses (Beck, p.75).

According to FDA documents and regulations and the Food and Drug Journal, just because the indications of use were approved by the FDA: “the FDA never has had authority to regulate the practice of medicine; physicians may use legally marketed drugs or devices in any way that they believe, in their professional judgment, will best serve their patients. Courts have repeatedly recognized the propriety of off-label use, and several states statutorily recognize off-label use in various contexts.”

The term investigational only applies when a new drug or device has been submitted for approval or when a manufacturer wants to market an approved drug or device for an off-label use.

The FDA regulates the marketing of approved drugs and devices but not the prescribing of those drugs or devices. Furthermore the Food and Drug Law Journal states the following:

“Off-label uses of medical devices and drugs perform an important therapeutic role in many, if not most, areas of medical practice.

Prescriptions for off-label uses of drug products “may account for more than 25% of the approximately 1.6 billion prescriptions written each year, with some recent estimates running as high as 60%.”

Pediatric uses also are mostly off-label. Thus, “in some cases, if you didn’t use the drug in the off-label way, you’d be guilty of malpractice.”(Beck, p. 80)

HBOT for brain injuries, for example, is simply the off-label use of a FDA approved drug and device.

THE PROBLEMS IS BECAUSE OF MEDICAL INSURANCE COMPANIES MISS USE OF THE LABELS OF FDA FOR NATIONAL COVERAGE OF INSURANCE OF MEDICINE TO MAKE MONEY NOT HYPERBARIC DOCTORS IN USA MOSTLY

The Food and Drug Law Journal further states the following in reference to off-label uses being neither “research” nor “experimental”:

The mere fact that a product is used off-label to treat a patient does not make that use research. As a matter of medical ethics, there is a distinction between the “practice of medicine” and “research.”

Certain third party payers have used the FDA label indications as a means for determining reimbursement, which, in practice, results in the untoward restriction of a physician’s ability to use an approved drug or device off-label.

Again, the FDA label was never intended to influence the practice of medicine, but the poor understanding of what the FDA was doing has been opportunistically used by third party payers to restrict reimbursement.

And finally, it is the practice of certain third party payers to use CMS National Coverage Indications (NCI) as the determining factor for reimbursement for any and all patients regardless of the age of the patient, and to call any indication not on this list as either research or an experimental indication.

Doing so is misuse of NCIs which are only intended to apply to 65 year old Medicare recipients and not, for example, Medicaid recipients.

SPECIALIZATION IN HYPERBARIC MEDICINE AND ITS LEVELS

Classification of Hyperbaric Medical Specialization Courses :

Hyperbaric Medical Course are Divided into Following Levels of Courses :

A: Level -1 course

A: Level -1 Courses :

The Level -2 Course are conducted Near the Hyperbaric Chambers and , also near the Sea and Diving Activities Station for 1 Year and then on Diving Ship like a Mine hunter etc

Deep Sea Diving is Conducted under Supervision of Trained Supervisors of Diving and also other Trained Hyperbaric Medical Physician Instructors as in Like NUMI .

Hyperbaric Medicine are discussed as Patient are Under going Treatment in a Hospital Based Environment where the Cases are Under going Treatment

Hyperbaric Medicine Training is About 3 Years Total in NUMI , that is Naval Under water and Medical Institute in Karachi , which has following Components :

6 months of Theory Part at NUMI Naval under water Medical Institute, under Bahria Naval University Department of Medicine and Naval Medical Directorate .
Plus 6 Months of Hyperbaric Medicine Training at PNS Shifa 1000 Bed Post Graduate Medical Institute , under Bahria Naval University Department of Medicine and Naval Medical Directorate , Clinical Aspects of Hyperbaric Medicine and its training under Senior Instructors.
1 Year diving and Practical Training at PNS Iqbal Naval Special Service Facility.
1 Year diving and Practical Training on Board a Diving Mine Hunter ship that has Active Diving Operations

Total of 3 Years of Course is done before a person is Fully Qualified as Hyperbaric Medical Specialist is Requirement in Pakistan Navy with both Theory and Practical Aspects.

Certificate of Hyperbaric Medicine Specialization is Issued by the Naval medical Directorate of Pakistan Navy in Islamabad , of 6 Months Duration only that only Signifies only the Theory Aspects and does not Mention the Practical 2 years and Six Months of Training that is a Flaw with Certificate ,does not Mention the, 6 months of Hyperbaric Training at PNS Shifa Hospital and also Practical Training of 2 Years that are Pre-requite for being Hyperbaric Medical Specialist which is an Anomaly that has to be Corrected the Certificate Issued by Pakistan Naval Medical Directorate , is of level -2 in International Classification.

The Level -2 Hyperbaric Medical Certificate that is Given by Naval medical Directorate of Pakistan Navy, of 6 months Duration Mentioning the Theory Part only as First Anomaly.

NUMI and Naval Medical Directorate also does not Mention the Training Part and Deep Sea Diving Portion of 2 Years or so apart from 6 Month Theory Certificate Issued by Naval Medical Directorate as Second Anomaly.

Also needing Correction, as the Defense Forces of Pakistan as Pakistan Navy and the Army medical Corps of Pakistan Army did not Pay the Fee for Registration to PMDC- Pakistan Medical and Dental Council , to Recognize Hyperbaric Medicine as a Master Degree of 3 Years Duration as Training and Theory is more than 3 Years .

The NUMI and Naval Medical Directorate and PMDC should at Least consider it as Diploma of 1 Years at Least as Fourth Anomaly , as Below 1 Years is not Considered as Diploma of Specialization and it is not even Printed on PMDC Licences, and this Problem Exists with Aviation Medicine Certificate as well from Pakistan Air-force .

For Instructors Advance Hyperbaric Course is done abroad , in USA and Australia when the Senior Hyperbaric Medical Specialists are sent by the Pakistan Navy after they complete the and Plus 3 Years of Training Post Qualification .

After this Training in USA and Australia the Doctor is now Consider Fit for being a Qualified Hyperbaric Specialist and Senior Instructor and he/she can be:

Examiner of Hyperbaric Medicine for Divers and Submariners
Instructor at NUMI , Naval Medical Under Water Medical Instructor.
Licensed to Practice as Specialist as Hyperbaric Medical Consultant

C: Level -3 Courses :

Level- 3 is a Post Graduate or Doctorate course in Hyperbaric Medicine , like the following In Pakistan as Level -3:

1. Master
2. PHD Degree

3. FCPS Hyperbaric Medicine from CPSP Pakistan

These level -3 are not Available as of yet , from Pakistan or even Abroad in English Speaking Countries and top Developed countries of the world.

Internationally Advance Level -3 , Courses are not Available in USA Europe, Asia Africa, or Oceania ( Australia / New Zealand ) in any English Speaking Countries.

Pakistan is trying to Introduce Master Degree course from Bahria University or NUMS , National University of Medical Sciences soon.

In Non English Countries , Egypt has a Master Degree from National Defense University is in Arabic Language.

The USA Some Doctors have Done ABEM American Board Emergency Medicine Specialization , and then Completing 1- 6 months Certification in Hyperbaric Medicine are Erroneously claiming to be American Board Certified Hyperbaric Specialist or Post Grads and are miss Understood as such.

These Senior Instructors and Consultant Doctors are Trained in Advanced Hyperbaric Medicine , from USA and Australia in Advance Hyperbaric Medicine course , and they are than Ready to Impart training to the New Doctors for Hyperbaric Medicine .

The Senior Instructors have More than 5-10 Years of Experience in Hyperbaric Medicine as a Specialist and have also Served for At Least 1 Year on a Deep Diving Ship also Known as Mine hunter in Pakistan Navy as it does Diving Operations of Deep Sea for Sea Based Mines and Weapons .

For Example I served for more than a Year on PNS Munsif a Mine Hunter ship that was doing Deep sea Diving and this a Special Ship that it has a Hyperbaric Chamber on Board also Known as TRCS Transportable Re-compression Chamber System , that joins with Bigger Hyperbaric Chamber via NATO Flange.

Only Mine Hunters has Hyperbaric chambers on Board and no other ship has Hyperbaric chambers on Board and this Kind of training can only be done on Mine Hunters.

PNS Shifa :

PNS Shifa , is A class of hospital with more than 1000 Bed and is based in Karachi and is Tertiary care A Grade Hospital with a lot of Specialties and Hyperbaric Medicine also comes under it and it has a Training Section that Trains the Nurses and Doctors as well and it is Under the Naval Medical Directorate and the Bahria University.

Pakistan Navy started PNS Shifa as a Hospital and a training school named as sick berth attendants in 1953. It was re-designated as “Medical Assistant Training School” in 1970.

The name of Pakistan Navy Nursing School was conceived in 1982 when the Female Nursing Cadets and Armed Forces Nursing Services (AFNS) trainee officer were also attached for various Professional courses.

Recently; keeping in view the role of hospital / school in imparting post-graduation training to medical officers, the name has been changed to Pakistan Navy Medical Training School.

PN Medical Training School is entrusted with the primary task of training to medical branch personnel of Pakistan Navy who serve on board ships / establishments to look after the patients.

This school is recognized by Sindh Nurses Examination Board & Sindh Medical Faculty and providing training facilities for General Nursing diploma course of 03 years duration and one year Operation Theater, Intensive Care and Midwifery specialization courses for AFNS officers.

The Hospital and its Training school also conducts house job, FCPS-II/DCPS (post-graduation) and Underwater Medicine Training.

Training School / PNS SHIFA is recognized by Pakistan Medical & Dental Council and College of Physicians and Surgeons Pakistan for House Job, Post-Graduation training in almost all the Disciplines of Medicine and Surgery and

Training in these Specialization is also being offered to Civilian Doctors in addition to Commissioned Medical Officers of Defense Forces.

Underwater Medicine Course is being offered to Defense Services Medical Officers of Allied Countries as well.

The Bahria University and NUMS National University of Medical Sciences :

Bahria now has its own Degree Awarding Institute known as Bahria University and has a Medical College under it in following Places :

Hyperbaric Medicine Training is About 2 Years Total which has following Components :

6 months of Theory Part at NUMI Naval under water Medical Institute, under Bahria Naval University Department of Medicine and Naval Medical Directorate .
Plus 6 Months of Hyperbaric Medicine Training at PNS Shifa 1000 Bed Post Graduate Medical Institute , under Bahria Naval University Department of Medicine and Naval Medical Directorate , Clinical Aspects of Hyperbaric Medicine and its training under Senior Instructors.
1 Year diving and Practical Training at PNS Iqbal Naval Special Service Facility.
1 Year diving and Practical Training on Board a Diving Mine Hunter ship that has Active Diving Operations

Examiner of Hyperbaric Medicine for Divers and Submariners
Instructor at NUMI , Naval Medical Under Water Medical Instructor.
Licensed to Practice as Specialist as Hyperbaric Medical Consultant

In Pakistan Level -3 , FCPS Hyperbaric or Master or PHD Degree is not Available as of yet , from Pakistan .

Internationally Advance Level -3 , Courses are not Available in USA Europe, Asia Africa, or Oceania ( Australia / New Zealand ) in any English Speaking Countries , Level -3 Degree, is not Available in any English Speaking Countries.

Hyperbaric Medicine in Pakistan Army for High Altitude Adventures :

Hyperbaric Medicine was Utilized in Late 1980,s in Pakistan Army for High Altitude Adventures and Early 1990,s when the War on Kargil which is highest Battle filed in the World and Pakistan and India has a cold War like Situation there since then and Active Warfare is being engaged on the Roof top of the World .

The Army Medical Corps Doctors were Involved in Solution of Problems of High Altitude Sickness in the form of HAPE and HACE known as High Altitude Pulmonary Edema and High Altitude Cerebral Edema .

The use of Flexible chambers were done to Save countless Lives of Army Personal involved in War Fare on the Roof top of the world as 150 Mountains exists that are above 7000 meters over there which is like 7 Km in Height from Sea Level .

K2 Mountain 8000 meters Tall one of Second Tallest in the World

Two of Top Most Mountains in Height like K2 and Nanga Parbat also are Situated over there in Karakorum Range of Hindu Kush Mountains that Divided Pakistan and Afghanistan and forms part of Durrand Line and also in the North where it Meets china border and also Indian Border where the Siachin Glacier at the Border of India Pakistan and china , where the Indian and Pakistan Army meets face to Face .

Hyperbaric Medicine Was used by the Army medical corps Doctors for the Saving of Lives and Currently it is also being used in Mountaineering .

As Hyperbaric Chambers are saving Lives of Mountaineers and also , Enabling them to climb the Mountain without using Oxygen Cylinders which are not helpful in Rare Atmosphere .

The Hyperbaric Chambers provides them at Base camp to Boost up the Oxygen Levels and Boost them to climb the Mountains and conquer the Challenges .

Pakistan Hyperbaric Specialists are Working in the Following Foreign Countries :

These Doctors are Working on Deputation and in Uniform mostly on Loan from Pakistan Army and Pakistan Navy and some Retired Doctors from Pakistan Defense Forces are also Working in Gulf countries.
