History of Hyperbaric Medicine in Saudia Arabia :

Saudia Arabia has been using Doctors from Pakistan Navy for Hyperbaric Medicine Needs of Navy and the Coast Guard.

The Saudi Air force and Army has also been Using the Hyperbaric and Aviation Medical Specialists from Pakistan for the Defense Needs of Saudia Arabia and Also the Special Services Group of Saudi Navy , also Known as Navy Commando,s also use the Pakistani Doctors .

Saudia Arabia has also recently sent its own Doctors to USA and has trained some of them and they work in Coast Guard Hospital and also in  Riyadh and  Jeddeh Tabuk Area of Saudia Arabia .

Hyperbaric Medicine in Saudia Progressed in late 1990,s when Saudia Spent a lot of money and made Jubail Naval Base and its Hospital has a Hyperbaric unit that is center of Hyperbaric Operations of Saudi Navy and Defense fores and there is a Hyperbaric Unit like the Haux Quadro and also a Second Circular Chamber as well is Situated in Jubail Naval base . about 4 Pakistan Military Doctor's as well as one Saudi Doctors Serves in the Hyperbaric Unit of Jubail Naval base Hyperbaric Unit .

The Saudi Defense Forces Doctors of Saudia : 

USA and Pakistan helped the Hyperbaric Medicine in Saudia Arabia and they were able to get some Doctors to be Hyperbaric medicine Qualified like Saudia Dr Yousaf al Oufi who is Director of Hyperbaric Operations in Saudia Arabia Jubail Naval base and he is In charge and has Degree in Hyperbaric Medicine from USA .

Saudi  Navy to do the Submarine and Naval SSG Special Services also Known as Naval Seals Operations take place from Jubail Naval Base , and  also the Submarine Warfare and X -Craft Spy and Tactical Under water Submarine Crafts that Operated under the Water, also Take place from Jubail . 

There is one or two more Doctors also in Saudi Defense Forces in Riyadh and Jeddeh of Saudi Nationality who are Trained from USA in Hyperbaric Medicine . 

The Pakistan Army Medical Corps Hyperbaric Specialist Doctors:  

Pakistan Navy Sent in the Doctors who were from Army Medical corps and they Trained them in Naval Seals / Navy SSG Operations in Karachi at Pakistan Navy Facilities and when they Got Trained they were Taught Hyperbaric Medicine in Institutes of USA in Co-operations with Bethesda Washington DC , and its Associated Hyperbaric Training Institutes along Side with USA Doctors of its Navy and Army . Some of them also did Navy Seals courses trained from NUMI , or Naval medical Institute Currently Serve in Jubail Naval base.

Many Doctors serve in the Jubail Naval Bases who are on Deputation from Pakistan Army and they Serve in Jubail Naval base and also in Jeddeh and Tabuk Army units of Saudi Navy and Air force where ever the chamber are Situated .

QUADRO MULTI PLACE CHAMBER 

Multi Place HBOT Chamber 

Multi place chambers are used by People in Hospital  , or Research Centers like Medical Universities or in Stand alone Emergency centers that are associated with a lot of Satellite Specialized Medical  clinics or Hospitals .

Hyperbaric Chamber are also being used in Plastic Surgery and Hair transplant , as well other Surgeries that required healing like Cardiac and Surgical orthopedic ones

Further more as Multi Place Diving Chamber are found near the Seas , where there is a  lot of Diving Activities are occurring like in the Navy and Special Forces units of Navy or the Army.

The Stand Alone Emergency Dealing Centers like the Coast Guard and the Navy also have their Own Multi place Hyperbaric Chambers .

The Oil and gas Drilling Industry also Deal in Hyperbaric Chambers and they are used in Deep Sea Diving and off Shore Drilling Operations

The Multi place Chambers are also found in Aviation and Space Technologies like in NASA and European Space Program's and in Future Pakistan can also use this for its Astronauts and Pilots .

The Chambers are Classified in North America by FDA and NFPA, that is Food and Drug Authority and National Fire Protection Agency

FDA USA Classification : Class 2 Chamber

NFPA USA : Class A

Hence it is Class 2 A Product

Both The FDA and EC in USA and Europe does Recommend the Hard Multi Place Chambers as worthy of Use for Regular Treatment of Hyperbaric Therapy,

Multi place Chamber Types : 

According to External Shape it has following types  

A. Cylindrical Multi-Place Spherical ( Cylindrical with a Bases ) 

CYLINDRICAL MULTI CHAMBER 

there is no Base at the bottom and a Special base has to be made at the bottom so that Chamber can be housed there either as trench or as a Plat Form so that Chamber is Securely Placed there .

B. Omega Multi-Place ( Does not Require a Base to Support Itself ) 

OMEGA MULTI PLACE HYPERBARIC CHAMBER 

Has a Flat Base at the Bottom which supports the Chamber and does not Require the Making of base for its installation and is very Easy to Install and Supports itself on a Flat ground and is very Good as Manufactured by Hipertech in Turkey who has the Patent for the Chamber .

C. Quadro Multi-Place (Rectangular)

QUADRO CHAMBER 

Rectangular Better system for lying down , Non Ambulatory Patients to go in via a hospital Trolley and Easy for Disabled Patients with sliding Doors or Hinge Doors.

Quadro has option for treating Lying Patients better than sitting Patients and it helps in healing them with help of qualified Nurse inside of the Chamber . 

Gas used to Pressurize Mono Chamber :

Multi Place Hyperbaric chambers are Classified according to Gas used for Pressurization : 

1. AIR Pressurization compression system 
2. OXYGEN Pressurization compression system 

Air Pressurization System : 

Just like the Multi place chamber has a AIR pressure and compression system , as AIR has 21 % oxygen only and rest is Nitrogen 78% an inert gas That does not take part in combustion or explosion , and hence safer then Oxygen which is highly combustible .

However the Oxygen Compression Multi place Chambers are also manufactured by Different Companies The Air Compression Multi place chamber is more expensive than the Oxygen compression system in price .

In the Air compression oxygen mask or oxygen helmet is used to deliver the oxygen to the patient under taking the treatment .

Oxygen Pressurization System :

In the other pressure compressor system where the oxygen is used for treatment although cheaper change in price and no oxygen mask is used for the patient and he / she lies just like that in the chamber.

But there are drawbacks to This.chamber as it can cause an explosion as pure oxygen is being used for treatment of the patient . Pure oxygen is 100% saturation . This chamber is relatively more dangerous than the Air compression system in mono chamber and most of Mono chambers have this system all around the world and Air compression in Mono place HBot chamber is in un common .

Control System to Run the Chamber : 

There are two Types of Control Systems : 

1. Fully Computerized Control System 
2. Manual Control System 

Computerized Control System : 

In Fully Automatic HBOT Chambers Software's Control the Multi Place Chamber and its Prevents the  Barotrauma by Controlled Pressure Increase and Depth Control , and hence is more Beneficial for the Patients and it is more conformable to run and it corrects the Mistakes of Operator who can Injure the Ears and Sinuses of Patients by controlling the Depth and Pressure Control . 

Manual Control System : 

In Manual Controlled HBOT Chambers Operator Control the Multi Place Chamber via a Handle like Throttle shaped like Airplane Jet Control , hence it is more Direct Control and Mistakes are not Corrected by the HBOT Chamber. A well trained Operator is Required for Operation and running of this Chamber with Sufficient Experience .

Hyperbaric Medical Associations and Operating Pressures : 

UHMS of USA ,European EUBS , AHDMA , Asian Hyperbaric Medical Association and Oceania South Pacific Region , SPUMS South Pacific undersea and Medical Society also Agrees with USA and EC standards and it considered as safe and reliable Medical Equipment and Included as part of HBO Treatment Options that are recommended by Doctors and the hyperbaric Associations as Multiplace Hyperbaric Medical and Diving Chambers :

Operating Pressures to treat up to 4 Bars Pressure for Medical Multi place Chamber and up to  10 Bars for Diving Multi Place Chamber and more than that for Saturation Multi Place Diving Chamber used in Oil and Gas exploration.

OXYGEN HELMETS

Advantages of Multi place HBOT chambers :

A:  Safety Features: The Multi place HBOT chambers have the following:

1. Fire Suppression system FSS , for  Fire and explosion there is safety system that can Extinguish the fire and save the life of patient like Sprinkler system. 
2. Escape features , the chamber has the Escape Mechanism in case of Emergency and Features exist that can open the chamber from inside in case of Emergency. 
3. Highly Safe as compared to all the Chambers  

B: Oxygen Via Mask and Helmets for Consumption Directly.

1. 100% Oxygen is used via the BIBS System  as Pure Oxygen System for Breathing via Mult Place Chambers attached to the Oxygen Cylinder  or Liquid oxygen System enables Safety of patient , and also that can lead to Fire Hazard as Oxygen is Inflammable gas . 
2. Because of Safety of Breathing Non Pressurized Oxygen it is Best and Safest system 

C: Air Compression System.

1. In Quality Multi Place HBOT Chambers using Air Compression System it is a lot Safer that has only 21% Oxygen in the Compression System as Air that has 72 % Inert Nitrogen Gas in it that has less Chance of Combustion and Catching Fire.  
2.  In Oxygen Compression System Oxygen can escape from from the Defective Seals who are not Maintained and Replaced every Year at least as it is prone to Wear and Tears , Leading to accidents and therefore the Multi place Hyperbaric System is best as it used Air for compression . 

D: Patient Comfort in Multi-Place HBOT Chambers.

1. Air- conditioning is very big Factor and as Gas is Compressed and Molecules come together of Gas inside of chamber it cause Heating up of Inside of Chamber and in Summers and Hot Weather it is like sitting in a Oven . Hot Environment inside of Session leading to Excessive sweating and Dehydration of patients as Patient has to be for one hour to 2 Hours at Least inside of HBOT chambers . in Multi Place Chambers there is in Built Air Conditioning system for temperature and Humidity.  
2. Multi- Media is Provided for the Patients so that , the Long Session is not boring with Multi media System that is Specially Sealed Electromagnetically and does not cause Accident as Normal LED and Multimedia would not function inside of the Chamber . 

E: Attendant and Medical Nurse requirement Issues  :

1. Medical Nurse can be inside of Chamber and one has to be outside on Controls and the one inside can intervene in case of Emergencies inside of Multi Place Chamber . 
2. The Patients who Need constant care and are Aggressive and Children are easily handled inside of Multi Place HBOT Chamber. Attendant cannot go Inside of Mono Place for the Children as they cant be Alone inside of Chamber as they feel Suffocated and Terrified and Claustrophobia can be an issue with Children as well as Adults especially with Hyper Active children and requiring Special Care .are handled well in Multi Place Hyperbaric Chamber.

Pakistan Has 150 Mountains More than 7000 Meters High and World 2nd Highest as K-2

1. LAST CHALLENGE TO CONQUER IN WINTER K2  SECOND HIGHEST IN WORLD MOUNTAIN  : 

Will the most dangerous mountain of Karakorum and the planet ever be climbed in winter? In these days, two teams proceed in parallel on the Sperone degli Abruzzi to try the business

Two expeditions are currently on the K2 (8,611 m) to attempt the climb of this eight thousand, so far inviolate in the coldest season. If they complete the impossible task, they will have solved one of the big problems of mountaineering: the first winter ascent of the second highest mountain on the planet.

"You can not imagine how much more difficult it is to climb the K2 in winter than spring or summer," explains Basque Alex Txikon to Andrew Bisharat of National Geographic , in an interview from the Base Camp (4,960 m) . Txikon is the head of the Spanish expedition that also includes Polish and Nepalese climbers. The other team is made up of a group of elite alpinists from Russia, Kyrgyzstan and Kazakhstan, led by Vassily Pivtsov . "It was a big challenge to reach only the base camp , " says Txikon, referring to the 60-mile trek to get to the 8,000 feet, through rugged snow-covered mountain valleys.

The expeditions are working on the Spur of the Abruzzi , the way chosen by both teams to reach the summit. They are equipping the course in parallel and setting up the high fields. Currently everyone is at the CB to regain strength and looking forward to more favorable weather conditions.

The K2, which straddles the border between Pakistan and China, is about two and a half meters lower than Everest, but is considered the hardest and most dangerous mountain on the planet. Unlike the "roof of the world", all sides of the K2 are extremely steep and subject to frequent falls of boulders and avalanches. Among the major obstacles, the infamous "Collo di Bottiglia", a very dangerous couloir, at about 8200 meters, where in 2008 11 alpinists lost their lives following the collapse of a serac.

Of the 14 mountains over 8,000 meters, the K2 remains the only peak still untouched during the winter. "I can not say it's the last challenge on the 8,000," says Txikon. "But it's one of the last great challenges."

Txikon is one of the most experienced and ambitious mountaineers in the world. In 2016, with Simone Moro and the Pakistani Ali Sadpara, he completed the first winter ascent of Nanga Parbat (8,126 meters), a mountain attempted more than 30 times in the winter, before being won.

2. IGLOSS SAVES FROM EXTREME COLD  

Txikon's team at the K2 CB built three large igloos. Inside, the temperature is 10-15 degrees higher than in the tents, moreover they offer a greater shelter from the exasperating noise of the incessant winds. According to Basque, the best rest conditions offered by the igloos could facilitate recovery and allow better performance.

With Txikon, there is Galician Felix Criado. The two mountaineers are accompanied by eight Sherpas, including Nuri Sherpa, Hallung Sherpa, Chhepal Sherpa, Geljen Sherpa and Pasang Sherpa .

"Sherpas are much stronger than many known climbers," Txikon says ironically. This group took part with the Basque in two consecutive winter expeditions on Everest, in the last two years, which were not successful. However, they have accumulated a lot of experience, climbing together in extreme conditions. Having been rejected twice by Everest in winter, this year the team wanted to try a new goal to keep motivation high.

The team also joined three Poles Paweł Dunaj, Marek Klonowski and Waldemar Kowalewski . Climbing 8,000 in winter has now become a Polish national hobby; just think that 10 of the 13 winter climbs on the 8,000 were made by Polish mountaineers.

Unfortunately there were some defections: Kowalewski, who joined the expedition last, was the victim of an injury, hit the collarbone by a rock while descending to Camp 1 and left the expedition in recent days, as well as Marek Klonowski, who has retired due to a heart problem.
Two other members of the Txikon team, Jon Barredo and Eva Robles (the only woman in the expedition), both in the approaching trek (non-mountaineers), were evacuated by helicopter because Barredo had knee pains and Robles decided to accompany him .

The Spanish-Polish-Nepalese team arrived at the base camp on January 16th and on the 28th of the month, and had already equipped up to Campo 2, at 6700 meters , on the Sperone Abruzzi, the normal route.

Next to the Txikon team, exactly parallel to the Spur of the Abruzzi, is the expedition of mountaineers led by the Kazakh Vassily Pivtsov , one of the few mountaineers in the world to have all 14,000 8000s , reached without additional oxygen. Txikon claims that the teams are not in direct competition with each other, although probably every mountain climber hopes to become the first to reach the summit of K2 in winter.

The team led by Pivtsov arrived at the base camp on January 15 and brings together some of the world's highest altitude mountaineers, including the Kazakhs Dmitriy Muravyov and Tursunaly Aubakirov ; the Russians Artem Braun, Roman Abildaev, Konstantin Shepelin ; and Kyrgyz Michael Danichkin . The group struggled to find a sponsor, so they raised funds through a crowdfunding operation and selling t-shirts through their Instagram account.

Pivtsov's team has fixed Txikon's independent ropes and is making rapid progress. Pivtsov and Braun set up Camp 2 at 6800 meters of Sperone Abruzzi, where they spent a night to acclimatise.

Adam Bielecki, who last winter took part in the failed Polish expedition on K2, recognizes the team's skills "Do not go to K2 in winter if you are not a true mountaineer" .
However, he believes that the chances of success are scarce: "It's a really difficult mountain to climb in winter - he says - it's the worst place on Earth. No team has ever exceeded 7,650 m , and with the top at 8,611 m, those last meters will be very tough. "   cold and wind, among the greatest dangers

Cold is one of the most dangerous challenges to face; on the mountain the temperatures drop to -65 °C. In such conditions, even the smallest mistake can have catastrophic consequences: a fallen glove can lead to freezing in a few minutes.

But the wind is the true killer, one of the greatest difficulties encountered in winter compared to the summer season. In winter, the Karakorum group, where K2 is located, is subject to stronger currents than the Himalayas. The gusts can reach the strength of the hurricanes and "snatch" the climbers from the mountain walls in an instant. This explains in part why the 8,000 in Nepal, including Everest, were the first mountains to be conquered in winter, while more than 20 years have passed before the eight thousand of Karakoram were taken into account as winter targets.

Another significant factor of difficulty, which is added in the winter on the K2, is the little snow present on the mountain. The strong winds sweep it away, leaving behind naked rock and friable ice that makes the climb considerably more technical and slower, exposing the climbers to the cold cold for longer periods.

Winter is also a period of low pressure, which translates into less oxygen expenditure than summer high-pressure climate windows. None of the climbers plan to use oxygen to reach the summit, although the Txikon team is carrying oxygen tanks in case an emergency occurs that requires a rapid descent.

Cory Richards, National Geographic photographer and the only American to have climbed an 8000 in winter, the Gasherbrum II (8,035 meters), with Denis Urubko and Simone Moro, in 2011, says: "All the difficulties of climbing a 8000 during the summer season must be multiplied by 10. "

" Climbing an 8000 in winter is not fun," explains Bielecki . The strong motivation that drives mountaineers is "the satisfaction of doing something that no one has ever done before - assuming it is possible to do so."

According to Txikon, to be successful on K2 "you have to be smart, fast, strong enough, have respect for the mountains, for these remote places and for the local population. You must have empathy and a great team. Never forget to be patient, and always remember that true success is coming back alive ".

Meanwhile, time is running out , The time limit for an undisputed winter ascent is 28 February 2019, the last day of the meteorological winter.

However, some mountaineers, including Txikon, argue that a summit reached before March 20, the end of the astronomical winter, would still apply.

Last Sunday, Dmitry Muravjow, Konstantin Szepelin and Mikhail Danichkin, whose conditions seem to have improved, transported some necessary equipment and supplies to the K2 Advanced Base Camp , before returning to base at night. The climb was also intended to improve the conditions of the route to the ABC.

The weather is still not good to work in the high parts of the mountain, but apparently the team of Pivtsov and Brown wants to make the most of the next window of good weather. It seems, in fact, that they are preparing for a difficult push on the mountain.

4.  HIGH ALTITUDE PULMONARY EDEMA & CEREBRAL EDEMA  TREATMENT BY HYPERBARIC CHAMBER 

Meanwhile, the Russian-Kazakh-Kyrgyz expedition is being prepared with a Potable Hyperbaric chamber treatment ( Gameo Bag ) 

The last days at the base camp have been used to regenerate the body (taking care of the cough) and to regain its strength. The team received a new supply of drugs in recent days and each member of the expedition also underwent an experimental "treatment" in the Gamow case, a "portable hyperbaric chamber" designed by Igor Gamov (portable hyperbaric bag - Gamow bag), very useful device in high-altitude shipments.

It is usually used to combat the severe symptoms of altitude sickness. The principle of its operation is very simple: the air is introduced into the chamber by means of a mechanical pump. The person inside the Gamow Bag is thus subjected to a greater environmental pressure, compared to the outside, comparable to a rapid descent up to 1500-2500 m. In this way, the concentration of oxygen increases in the air.

However, this treatment raised some concerns. If the mountaineers claim to reach a peak without additional oxygen, ie without artificial help to reduce the effects of high altitude, can they use a hyperbaric chamber at the base camp? Polish magazine Wspinanie talked about it with mountaineer Krzysztof Wielicki, asking for his opinion:

"The use of the" Gamow bag "by the Pivtsov team is a bit strange, because I have always treated this equipment as an emergency solution, used only in extreme situations - explains Wielicki - However, I would not criticize or condemn the method, because it is only one of the small facilities, like other various aids used in the high mountains. Air Pressurization System :

Oxygen Pressurization System :

In the other pressure compressor system where the oxygen is used for treatment although cheaper change in price and no oxygen mask is used for the patient and he / she lies just like that in the chamber.

But there are drawbacks to This.chamber as it can cause an explosion as pure oxygen is being used for treatment of the patient . Pure oxygen is 100% saturation . 2. Lack of Escape features , the chamber lacks the Escape Mechanism in case of Emergency and no Features exist that can open the chamber from inside in case of Emergency except the Zipper that can be opened from inside . 
3. Highly Dangerous and prone to Fires and Most of Fires reported are in Flexible chambers 

B: Oxygen is Used for both Compression and also Consumption Directly.

1. 100% Oxygen is used via the compressor System  In Pure Oxygen Mono Place Chambers attached to the Oxygen Cylinder that can lead to Fire Hazard as Oxygen is Inflammable gas . 
2. In Quality Mono Place HBOT Chambers using Air Compression System it is a lot Safer that has only 21% Oxygen in the Compression System as Air that has 72 % Inert Nitrogen Gas in it that has less Chance of Combustion and Catching Fire.  
3.  Oxygen can escape from from the Defective Seals who are not Maintained and Replaced every Year at least as it is prone to Wear and Tears , Leading to accidents 

C: Patient Discomfort in Manual Mono Place HBOT Chambers.

1. Lack of Air- conditioning is very big Factor and as Gas is Compressed and Molecules come together of Gas inside of chamber it cause Heating up of Inside of Chamber and in Summers and Hot Weather it is like sitting in a Oven . Hot Environment inside of Session leading to Excessive sweating and Dehydration of patients as Patient has to be for one hour to 2 Hours at Least inside of HBOT chambers . 
2. For Controlling the Hot Environment Air-conditioning is Required outside of Flexible chamber and if not Adequate can lead to Patients especially children to Abandon the Treatment . 
3. In Case of Air-conditioning not Maintaining a Certain level of humidity Outside of Flexible chamber can lead to formation of condensation inside and outside of chamber and Leaking water etc from Compressor and Piping and lot of humidity inside of chamber . 

D: Attendant and Medical Nurse requirement Issues  :

1. Attendant has to go Inside of Mono Place for the Children as they cant be Alone inside of Chamber as they feel Suffocated and Terrified and Claustrophobia can be an issue with Children as well as Adults . 
2. Medical Nurse cannot be inside of Chamber and has to be outside on Controls and he cannot intervene in case of Emergencies inside of Mono Place Chamber . 
3. The Patients who Need constant care and are Aggressive and Children are not Suitable for Mono Place HBOT Chamber. 
4. Not Really Comfortable inside of Mono place as compared to Multi place Chamber . 

E: Emergency Situation :

1. In case of Emergency the Mono Place HBOT chamber has safety Issues as it cant be Opened from Inside in case of Fire by the Patients and the Multi Place can be Opened from Inside by Manual Control . 
2. Cannot be Used for Diving Medical Emergencies as it is a medical Chamber and not Diving Chamber , Most people think they can use Medical Chambers for Diving Emergencies which is is a Fallacy and wrong Assumption as they are different Breed of Machines . 
3. Has no Fire Suppression System (FSS ) , Inside of Mono Place HBOT Chamber . 
4. Medical HBOT Nurse is Outside and he cannot Give Immediate Help in case of Medical Emergencies like Pulmonary Collapse , Heart Attack , or Convulsions etc and even Panic Attacks and Claustrophobic attacks etc and has to resort to Decompression to open up and then treat the Patient .
